DRIVING DIRECTIONS
From I-5 North and South: Take exit 252 (Kuebler Blvd) and turn west onto Kuebler Blvd. Drive 3.5 miles. Turn right at the stoplight onto Skyline Rd S.  Pain Management Northwest is the first driveway on the left.

From Downtown Salem: Drive south on Commercial Street 1.8 miles. Continue on Liberty Rd S towards Sprague High School/Rosedale - go 1.8 miles. Turn on Skyline R S - go .07 miles (just before reaching Kuebler Blvd) to Pain Management Northwest on your right.
